Insane Cultists are rather dangerous enemies that appear in Dead Marsh. While they're distant from the player, they like to toss pieces of their swollen brains, intoxicating the player, making the screen red and causing damage. At close range, they will try to punch the player with their giant fist. They also appear to use their giant arm to heal themselves while low on health, so kill them as fast as you can.
