Nurse[]
Nurses (or "Evil Nurses") are attractive female enemies (the opposite of the repulsive Doctors) that appear in Lab (Battle out of Hell and Hell Wars).
Nurses are, as the name suggests, tall female nurses with a sexy appearance: slim body, small nose, lipstick, long fingers with red polish, big breasts and curvy bottom; which is also complemented by their provocative attire: high-heel black boots, pantyhose, white nurse cap, and a white nurse uniform that shows a lot of cleavage which is fastened by a black strap at the waist; the uniform is also wet in the left areola region, and blood splattered on the right side. They have short white hair in an almost straight bob style, brown eyes; and their mouth is always agape, revealing their impressive white teeth. On her uniform, on the left side of the chest is written "Mona Sucks"; they also have a hidden red heart on the rear of their white undies. They wield a large unusual syringe of red liquid, with a handle that has "HIV" written on it.
When not alerted to the player, they patrol the area while walking sexily; however, as soon as they spot the player, they will start chasing them while giggling and laughing continuously, and not running much faster than before due to their high heels. When they are close to the player, they will inject them with the content of the syringe, though despite its appearance, it will not poison the player like other enemies.
The upgraded version of the Nurse appears during the ambush from behind in Lab (Battle out of Hell and Hell Wars); they are always alerted to the player's location, so hiding from them won't work.

Although they look friendly enough, the Nurses who staff the Secret Lab are not interested in keeping you healthy. If you get too close, you can expect to experience a world of pain from the poisonous syringes they wield.
Military Nurse[]
The Military Nurse is a scrapped version of the Nurse. Nothing is known about her functionality, and the only thing left of her is a concept art that can be seen during the "Making of" video, and in many levels as either painting, banner or graffiti. It is assumed that she might've appeared in Leningrad.

Managers are tall women with a sexy appearance: slim body, small nose, lipstick, long fingers with red polish, big breasts and curvy bottom; which is also complemented by their provocative attire: high-heel black boots, pantyhose, and a black uniform that shows a lot of cleavage which is fastened by a black strap at the waist; the uniform is very tightened on her body, which results in her nipples being shaped into the uniform, also the right side of her uniform is blood splattered. They have short white hair in an almost straight bob style, brown eyes; and their mouth is always agape, revealing their impressive white teeth. They also have a hidden red heart on the rear of their white undies. They wield a large unusual syringe of red liquid, with a handle that has "HIV" written on it.
They behave exactly as the Nurse, except that they are always alerted to the player's location and will come after them while giggling and laughing continuously; they don't run fast due to their high heels. When they are close to the player, they will inject them with the content of the syringe, though despite its appearance, it will not poison the player like other enemies.

- The Nurses and Managers cannot be gibbed, not even when frozen or petrified.
- Their dress is tightened on their legs when walking and running, but in any other circumstance, it is not.
- Their corpse takes longer to disappear(8 seconds) than other enemies'.
- The "Mona Sucks" inscription on Nurse's uniform is a reference to Mona Sax from Max Payne series.
Despite not appearing in this game, paintings, banners and graffiti of Evil Nurse's concept art and Military Nurse's concept art can be seen in many levels.
