Orphanage is the first level of Painkiller Battle Out Of Hell and one of the most memorable levels in game.
Description
This level is set in dark and haunted orphanage. The whole building is set on hill which is surrounded by swamp which water causes damage. Also on one of the small islands, which are scattered everywhere the swamp, there are small wodden crosses that belong to deceased children. Interior of the building is very dim. There are a lot of broken toys and blood stains everywhere and screams of childrens and infants can be constantly heard. Inside the orphanage is three storey library, playroom with broken toys and big bear with a torn stomach, classroom, bedrooms with cages that resemble torture room, dining room and kitchen. Below the orphanage is cellar and entrance to the crypt. Overall there is an atmosphere of terror and fear.

Tarot card of this level is Weapon Modifier. To acquire it, player must never get hurt.
Trivia
- If you use the iron will, double time bonus and speed you are actually able to get to the edge of the swamp of this level. You will not actually take any damage while you stay there but if you go more closer to the center you will be greeted with an instant game over. Note that only the right edge will not give you a game over, the others will give you a game over no matter where you stay.
